Elin işlevini bozan başparmak hipoplazileri ve tenar kas anomalileri kompleks konjenital hastalıklar olup, genellikle başka sendromlar veya konjenital anomalilerle birlikte görülürler. Abduktor pollisis brevis kasının konjenital yokluğu oldukça nadirdir ve genellikle başka kas, sinir ve tendonların yokluğu veya fonksiyon bozuklukları da bu kasın yokluğuna eşlik eder. Yirmi yaşındaki erkek hasta, ellerini tam kullanamama, özellikle başparmağıyla diğer parmaklarını kavuşturamama ve ince beceri gerektiren işleri yapamama yakınmasıyla polikliniğimize başvurdu. Fizik muayene, nörolojik inceleme ve elektromiyografi ile bilateral abduktor pollisis brevis kası yokluğu saptandı. Olguda herhangi bir düzeltici girişim yapılmasına gereksinim duyulmadı. Bu çalışmada, benzeri ulaşabilen yayınlarda bulunamayan sadece bilateral abduktor pollisis brevis kası yokluğu olan bu olgu sunularak, elin işlevini bozan konjenital hastalıklar ve tedavi yöntemleri gözden geçirildi.

Congenital absence of the abductor pollicis brevis muscle is very rare, and generally, absence or dysfunction of the other muscles, nerves or tendons will accompany this muscle’s absence. A 20-year-old male patient was admitted to our outpatient clinic with a complaint of deficiency in some hand functions; he especially could not touch the thumbs to the other fingers or do jobs that require such skills. Physical, neurological and electromyographic assessment revealed the bilateral absence of the abductor pollicis brevis muscle. For this case, no corrective initiative was needed. In this study, presenting a case with bilateral absence of the abductor pollicis brevis muscle, congenital muscle diseases determinant of hand function and their treatment methods are reviewed. To our knowledge, no such case report has been published in the literature previously.
