Mesane karsinomları, biyolojik davranış ve klinik seyir açısından heterojen karaktere sahip bir tümör grubudur. Tümörün tedaviye yanıtını ve hastanın prognozunu belirleme özelliği gösterebilecek histopatolojik parametrelerin araştırıldığı bu retrospektif çalışmada, transizyonel hücreli karsinom tanısı almış ve ortalama olarak 19.1 ay izlenmiş olan 64 hastaya ait bulguların analizi yapılmıştır. Bulgular, tümör derecesi ve evresinin yüksek prediktif değeri olan parametreler olduğunu göstermiştir. Orta derecede diferansiye olarak kabul edilen transizyonel hücreli karsinomların farklı biyolojik polansiyele sahip tümörleri içerdiği gösterilmiştir. Bu grubun 2A ve 2B diye iki alt gruba ayrılarak incelenmesi önerilebilir. Evre Ta ve T1 tümörler de rekürrens ve prognoz yönünden incelendiğinde önemli farklılıklar içerdiği görülmektedir. Tümörün solid paterne sahip veya multifokal olması prognozu olumsuz etkileyen özelliklerdir. Skuamöz diferansiyasyon gösteren tümörlerin tedavisinde radyoterapi önemli oranda başarısız olmuştur.
Trasitional cell carcinoma (TCC) of the bladder is a heterogeneous tumor group in respect to biological behaviour and clinical outcome. This retrospective study was planned to investigate the histopathological parameters which can help to predict the prognosis of the patient. 64 patients whose tumor was diagnosed as TCC, with a mean follow-up of 19.1 months were included in the study. Our results indicate that tumor grade and stage are the most reliable parameters in predicting the prognosis. Intermediate grade tumors are in fact a heterogeneous group which deserves subdivision for better control of the tumor. Stage Ta and T1 tumors also bear important differences in recurrence and prognosis. Besides, tumor growth pattern, multifocality, presence of CIS and squamous differentiation of the tumor play a considerable role in the tumor response to therapy.
