Orbital Fat Decompression in Graves Ophthalmopathy

The purpose of this study was to evaluate the efficacy and safety of orbital fat decompression and observe the short-term results. In order to decrease the amount of proptosis, fat was removed from the temporal and medial compartments of the bilateral upper eyelids via transcutanous surgery and transconjunctival surgery in the right lower lid of a 35-year-old man with Graves ophthalmopathy. Short-term results were reviewed at 1 week, 1 month, and 3 months. Thirty mm proptosis was measured preoperatively; 23 mm proptosis was measured in the right eye and 26 mm proptosis was measured in the left eye postoperatively. A 2 mm opening remained in the left eye with eye closure. Limitation of the upper eyelid in downward gaze and conjunctival and scleral conjection were all diminished postoperatively. With the exception of compressive optic neuropathy, decreasing orbital volume via orbital fat removal produces satisfactory results in regression of proptosis.

Graves Oftalmopatide Orbital Yağ Dokusu Dekompresyonu

Bu yazıda, orbital yağ dekompresyonu tekniğinin etkinlik ve güvenirliliği ve erken dönem sonuçları değerlendirildi. Graves oftalmopati tanısı ile takip edilen 35 yaşındaki bir erkek hastaya proptoz miktarını azaltmak için temporal ve medial kompartmandaki yağ dokusu, heriki üst göz kapağında transkutanöz ve sağ alt göz kapağında transkonjonktival yaklaşımla eksize edildi. Birinci hafta, birinci ay ve üçüncü ay erken dönem sonuçlar değerlendirildi. Her iki tarafta 30 mm olan ameliyat öncesi proptoz miktarı, ameliyat sonrası sağda 23 mm, solda 26 mm olarak ölçüldü. Gözler kapalı durumda iken sol tarafta 2 mm kapak açıklığı kaldığı saptandı. Üst göz kapağının aşağı bakış hareketindeki geri kalması ve konjonktiva ve skleradaki konjesyonun ameliyat sonrası azaldığı saptandı. Kompresif optik nöropatinin gelişmediği olgularda, orbita yağ dokusunun uzaklaştırılması ile orbital hacmin azaltılması proptoz miktarında tatminkar gerileme sağlamaktadır.
