Indications for keratoplasty operation and operation techniques

OBJECTIVE: The aim of this study was to evaluate the distribution of indications and the type of operation for keratoplasty (KP).
METHODS: Records of all patients who underwent KP, between January 2008-December 2008 were rewiewed retrospectively. The age, sex and laterality of the eye, indication for keratoplasty and type of the operation [penetrating keratoplasty (PK), deep anterior lamellar keratoplasty (DALK), penetrating keratoplasty (PK) only or concomitant surgical procedures] were recorded.
RESULTS: 239 operation of keratoplasty were included in the study. We showed that, most common indication was pseudophakic or aphakic corneal edema (62 eyes, 25.9%). Keratoconus is the second most common indication (61 eyes; 25.5%) and corneal dystrophy is the third most viewed (31eyes, 13%). 27 patient (11.3%) was regreft. Of the 239 operations, 201 (84.1%) were PK and 38 (15.9%) were DALK. Of the PK operations, 14 (7%) were combined with extracapsular cataract extraction (ECCE) and posterior chamber (PC) intraocular lens (IOL) implantation, 7 (3.5%) were combined with scleral fixated (SF) IOL implantation, 5 (2.5%) were combined with pupilloplasty, 7 (3.5%) were combined with synechiolysis.
CONCLUSION: Pseudophakic and aphakic corneal edema were the most common indication for keratoplasty with keratoconus. In addition keratitis scar, corneal distrophy, regraft and traumatic corneal skar were other common indications for keratoplasty.

Keratoplasti ameliyatı endikasyonları ve ameliyat teknikleri

AMAÇ: Bu çalışmanın amacı, keratoplasti (KP) ameliyatlarında endikasyonları ve operasyon tiplerini saptamaktı.
YÖNTEMLER: Kliniğimizde Ocak 2008-Aralık 2008 tarihleri arasında yapılan keratoplasti ameliyatları geriye dönük incelendi. Hastaların yaşı, cinsiyeti, hangi gözün ameliyat edildiği, endikasyonları ve operasyon tipi [Penetran keratoplasti (PKP), derin anterior lameller keratoplasti (DALK), tek başına keratoplasti veya beraber kombine cerrahiler] kaydedildi.
BULGULAR: Çalışmaya 239 keratoplasti hastası alındı. Hastaların en sık psödofakik veya afakik korneal ödem endikasyonuna bağlı ameliyat edildiği görüldü (62 göz, %25.9). İkinci sıklıkta keratokonus (61 göz, %25.5) ve 3. sıklıkta korneal distrofi (31 göz, %13) olduğu görüldü. Yirmi yedi hastada (%11.3) greft yetersizliği vardı. Toplam 239 ameliyatın 201’i
(%84.1) PKP ve 38’i DALK (%15.9) idi. PKP olanların 14’ü (%7) ekstrakapsüler katarakt ekstraksiyonu (EKKE) ve arka kamara (AK) göz içi lensi (GİL) implantasyonu, 7’si (%3.5) skleral fikse GİL implantasyonu, 5’i (%2.5) pupilloplasti, 7’si (%3.5) sineşiolizis ile kombineydi.
SONUÇ: Keratoplasti ameliyatları kliniğimizde en sık keratokonus ve psödofakik veya afakik korneal ödem endikasyonu nedeniyle gerçekleştirilmiştir. Bunların dışında keratit sekeli, korneal distrofi, greft yetmezliği ve travmatik korneal skarlar nedeniyle de keratoplasti sıklıkla uygulanmaktadır.
