Evaluatıon Of Patıent’s Complaınts After Laparoscopıc Nıssen Fundoplıcatıon

OBJECTIVE: Gastroesophageal reflux disease (GERD) is one of the most common encountered gastrointestinal system disorders. Laparoscopic fundoplication procedures have been popular with the introduction of laparoscopic operation to the surgical field. Although the degree of the symptoms relief correlates with the success of the surgical procedure, there is no method available to measure the relief of symptoms. The aim of this study was to measure the timing of symptoms relief after operation by using a scale in patients that laparoscopic Nissen fundoplication were performed due to GERD. Wound healing, recovery of intestinal and respiratory functions of the patients were evaluated at 2., 4., 6. and 8. week after the operation. The patients were asked to choose point from 0 to 10 on visual analogous scale according to the degree of symptoms. The scoring points were repeated the day before operation and at 2., 4., 6. and 8. week after the operation. The total score obtained before operation was 33 (±4.83). The total score obtained at 2., 4., 6. and 8. week after the operation were 19.7 (±8.7), 8.6 (±7.23), 3.7 (±5.13) and 1.8 (±3.46), respectively (p=0.001, p<0.01). The decrease rates of the degrees of the symptoms before operation were 22%, 57%, 77% and 87% at 2., 4., 6., and 8. week after the operation, respectively. The symptoms relief augments and continues until postoperative second month according our scale performed. Thus, we consider sharing this information with the patients preoperatively should improve the compliance of the patients to the treatment.

Laparoskopik Nissen Fundoplikasyonu Ameliyatından Sonra Hasta Yakınmalarının Değerlendirilmesi

AMAÇ:
Gastroözofageal reflü hastalığı (GERD) intestinal sistemin sık rastlanan hastalıklarından birisidir. Laparoskopik ameliyatların popüler olmasından sonra laparoskopik fundoplikasyon ameliyatları da popüler olmuştur. Semptomlardaki azalma ameliyatın başarısını göstermekle birlikte bu semptomlar arasındaki azalmayı ölçecek yöntemler yoktur. Bu çalışmanın amacı GERD nedeniyle laparoskopik olarak Nissen fundoplikasyonu uygulanan hastalarda ameliyat sonrası dönemde yakınmaların azalma zamanını bir skala yardımıyla saptamaktır. Hastaların ameliyat sonrası 2., 4., 6. ve 8. haftalarda yara iyileşmeleri, intestinal sistem ve solunum işlevleri değerlendirildi. Skala oluşturulurken hastalardan görsel analog skala üzerinde semptomlarının ağırlığına göre 0’dan 10’a kadar puanlar vermeleri istendi. Bu skaladaki puanlama, ameliyattan bir gün önce ve ameliyattan sonra 2., 4., 6. ve 8. haftalarda tekrarlandı. Ameliyat öncesi skladan elde edilen toplam puan 33 (±4.83) idi. Ameliyat sonrası 2., 4., 6., 8. haftalarda elde edilen toplam skorlar ise sırasıyla 19.7 (±8.7), 8.6 (±7.23), 3.7 (±5.13), 1.8 (±3.46) idi (p=0.001, p<0.01). Ameliyat öncesi dönemdeki semptomların ameliyat sonrası 2., 4., 6., 8. haftalardaki azalmaları ise sırasıyla %22, %57, %77, %87 oranında idi. Uyguladığımız skalaya göre hastaların semptomlarındaki düzelmeler artarak ameliyat sonrası dönemde iki ay kadar devam etmiştir. Bu bilginin ameliyat öncesi dönemde hasta ile paylaşılmasının hastanın tedaviye uyumunu artıracağı kanaatindeyiz.
