Schould glucose screening test be done in pregnant women 24 years and under?

OBJECTIVE: Diagnosed for the first time during pregnancy or manifesting itself during pregnancy, any degree of glucose intolerance is referred to as gestational diabetes mellitus (GDM). In this study, we aimed to show that when the glucose screening test is not applied in pregnant women under 24 years who are not in the high-risk group, no decrease in the diagnosis rate is seen, and further, that the National Diabetes Data Group (NDDG) and
Carpenter-Coustan criteria can both be used for GDM diagnosis.
METHODS: The files of 884 pregnant women who were seen at the obstetrics and gynecology clinic of our hospital during 2008-2009 were retrospectively studied. The results of the 50-g glucose screening test and the oral glucose tolerance test (OGTT) were evaluated.
RESULTS: While the ratio of those with two-value positivity among the pregnant women over 24 who were given OGTT according to NDDG criteria was 12.1%, this ratio was 6% for women under 24. These ratios for women who were given the OGTT according to Carpenter-Coustan criteria were 20.1% for over 24 and 12% for under 24. In the OGTT given to pregnant women under 24 for GDM diagnosis, there was no statistical difference
in the two-value positivity according to NDDG and Carpenter-Coustan criteria (p>0.05).
CONCLUSION: There is not yet a global consensus on the GDM screening test, the diagnostic threshold values for each test, or whether or not they should be applied to selected or all pregnant women. For the diagnosis, NDDG and Carpenter-Coustan criteria can be used.

Yirmi dört yaş ve altındaki gebelere glukoz tarama testi yapılmalı mıdır?

AMAÇ: İlk kez gebelikte tanısı konulan ya da gebelik sırasında ortaya çıkan, herhangi bir derecedeki glikoz intoleransına gestasyonel diabetes mellitus (GDM) adı verilir. Çalışmamızda, yüksek risk grubunda olmayan 24 yaş altındaki gebelere glikoz tarama testi yapılmadığında tanı oranında düşme olmadığını ve GDM tanısında Ulusal Diabet Veri Grubu (UDVG) kriterleri ile Carperter ve Couston kriterlerinden ikisinin de kullanılabileceğini göstermeyi amaçladık.
YÖNTEMLER: 2008-2009 yılları arasında hastanemiz kadın hastalıkları ve doğum polikliniğine başvuran
884 gebenin dosyaları retrospektif olarak incelendi. 50 gr glukoz tarama testi ve oral glukoz tolerans testi (OGTT) sonuçları değerlendirildi.
BULGULAR: UDVG kriterlerine göre 24 yaşın üstündeki OGTT yapılan gebelerde 2 değer pozitifliği taşıyanların oranı %12,1 iken, 24 yaş ve altında bu oran %6 idi. Ayrıca 24 yaş üstü Carperter ve Couston kriterlerine göre OGTT yapılan hastalarda 2 değer pozitifliği taşıyanların oranı %20,1 iken, 24 yaş ve altında bu oran %12 idi. GDM tanısında yapılan OGTT’de yaşları ≤24 olan gebelerde UDVG kriterlerine göre 2 değer pozitifliği ile Carperter ve Couston kriterlerine göre 2 değer pozitifliği olanlar arasında istatistiki olarak
fark yoktu (p>0,05).
SONUÇ: GDM tarama/tanı testi hakkında, her testin tanısal eşik değerleri hakkında ve seçici ya da tüm gebe kadınlara uygulanıp uygulanmaması hakkında henüz küresel bir konsensüs yoktur. Tanıda UDVG kriterleri veya Carperter ve Couston kriterleri kullanılabilir.
