The Role Of Gentamycin-Polymethylmethacrylate (G-Pmma) Chains In The Treatment Of Chronic Osteomyelitis

OBJECTIVE:
Although it’s rarely seen in developed countries, chronic osteomyelitis is still an important problem with its serious complications. The classical methods of treatment are systemic antibiotic therapy, saucerisation, bone grooving, continue irrigation-drainage, local muscle flaps and finally amputation. The aim of this study is to evaluate the effects of G-PMMA chains, which has been an alternative method instead of classical procedures. In our clinic, we treated 31 chronic osteomyelitis cases -26 (84%) male, 5 (16%) female- by embedding G-PMMA chains, between 1993-1999. The mean age was 40 (14-73). The chains were embedded in 13 (42%) femur, 10 (32%) tibia, 3 (10%) vertebrae, 3 (10%) pelvis, 1 (3%) humerus, 1 (3%) calcaneus. The major etiologic factors were found to be skeletal trauma in 24 (77%) and haematogenous osteomyelitis in 7 (23%). The mean follow-up time was 29 (4-75) months and it was more than 1 year for 77% of the cases. The success rates for at least 1 month and 3 months after treatment were found to be 93% and 87% respectively. Despite poor blood flow along the infection site, ability of reaching to high local antibiotic concentrations by G-PMMA chains is an important superiority. While achieving this, being away from toxic side effects, closure of wounds, early mobilization of patients, no necessities for re-operations, less need of systemic antibiotics, low cost are the other advantages. With all these properties, we believe that these chains will continue to be valuable in the treatment of chronic osteomyelitis.

Gentamisin Polimetilmetakrilat (G-Pmma) Zincirlerinin Kronik Osteomyelit Tedavisindeki Yeri

AMAÇ: Kronik osteomyelit günümüzde gelişmiş toplumlarda çok az görülmesine rağmen, ciddi komplikasyonlarıyla önemli bir sorun olmaya devam etmektedir. Hastalığın tedavisi için sistemik antibiyoterapi, oluklaştırma, tabaklaştırma, devamlı irrigasyon-drenaj, lokal kas flebi uygulaması, amputasyon klasik yaklaşımlardır. Bu çalışmanın amacı, klasik yöntemlere alternatif olarak geliştirilen G-PMMA zincirlerinin, kronik osteomyelitli olgulardaki etkilerini izlemek ve sonuçlarını değerlendirmektir. Kliniğimizde 1993-1999 yılları arasında tedavi edilen 26’sı (%84) erkek, 5’i (%16) kadın toplam 31 hastaya kronik osteomyelit tanısıyla G-PMMA zinciri uygulanmıştır. Yaş ortalaması 40 (14-73)’tır. 13 (%42) olguda femura, 10 (%32) olguda tibiaya, 3 (%10) olguda vertebraya, 3 (%10) olguda kalçaya, 1 (%3) olguda humerusa ve 1 (%3) olguda kalkaneusa zincir yerleştirilmiştir. 24 (%77) olguda hastalığın gelişmesinde ana etken iskelet sistemi travması, 7 (%23) olguda hematojen osteomyelittir. Ameliyat sonrası ortalama takip süresi 29 (4-75) aydır. Olguların %77’sinin takip süresi 1 yıldan fazladır. En az 1 aylık erken dönemde %93 ve en az 3 aylık geç dönemde %87 başarılı sonuçlar alınmıştır. İnfeksiyon bölgesinde kan dolaşımının ileri derecede bozuk olmasına rağmen, G-PMMA zincirleriyle yüksek lokal antibiyotik konsantrasyonu sağlanabilmesi önemli bir üstünlüktür. Bunu gerçekleştirirken toksik yan etkilerden uzak kalabilmek, yaranın kapatılabilmesi, hastaların erken mobilizasyonu, tekrarlayan müdahalelerin gerekmemesi, daha az sistemik antibiyotik ihtiyacı, düşük maliyet önemli diğer avantajlardır. G-PMMA zincirlerinin bu özellikleri ile kronik osteomyelit tedavisindeki etkili yerini koruyacağı inancındayız.
