A Comparative Study Of The Conservative And Surgical Methods In The Treatment Of Pediatric Forearm Fractures

Treatment of the unstable forearm fractures in children is very problematic and the limits of the conservative or surgical treatment are not obvious. In this study, comparison of the conservatively treated stable and unstable pediatric forearm fractures with the surgically treated unstable fractures was aimed. In the 8-16 age group, 7 stable and 26 unstable fractures were treated conservatively while the 18 unstable fractures were treated surgically. 17 of the patients were in 8-10 age group and the remaining 34 were in 11-16 age group. Following closed reduction long arm cast were applied to 33 patients, surgical treatment with intramedullary fixation or with compression plate were applied to the 18 patients in whom suitable reduction cannot be obtained. The mean follow-up time was 6 (4-9) months for conservatively and 13 (6-15) months for surgically treated patients. Satisfactory results were obtained in 20 (76.9%) of the 26 unstable fractures treated conservatively while the 6 (23.1%) had bad results. On the other hand, results in surgically treated patients were satisfactory in 15 (83.4%) and bad in 3 (16.6%). Malrotation was seen in 4 (22.2%) cases and loss of reduction in 1 (5.5%). Rate of satisfactory results in unstable fractures was better in surgically treated group than in conservatively treated group. In 11-16 ages group, malrotation rate was very higher in unstable fractures than in stable fractures. Rate of loss of reduction was very lower in surgical treatment. Closed intramedullary nailing seems to be the most suitable treatment modality, as the risk of complication is not high and having satisfactory results.

Çocuk Önkol Çift Kırıklarının Tedavisinde Konservatif Ve Cerrahi Yöntemlerin Karşılaştırılması

Çocukların anstabil önkol kırıklarının tedavisi sorunlu olup, cerrahi ve konservatif tedavilerin sınırları tam olarak belli değildir. Bu çalışmada konservatif tedavi edilen stabil ve anstabil çocuk önkol kırıklarıyla, cerrahi tedavi edilen anstabil kırıkların karşılaştırılması amaçlanmıştır. 8-16 yaş arası 51 hastanın 7’sindeki stabil önkol kırığı konservatif, 44 anstabil kırığın 26’sı konservatif, 18’i cerrahi olarak tedavi edilmiştir. Hastaların 17’si 8-10 ve 34’ü 11-16 yaş grubundadır. 33 hastaya kapalı redüksiyon sonrası uzun kol sirküler alçı, yeterli redüksiyon sağlanamayan 18 hastaya intramedüller fiksasyon veya kompresyon plağıyla cerrahi tedavi uygulanmıştır. Konservatif olarak tedavi edilen hastalar ortalama 6 (4-9) ay, cerrahi olarak tedavi edilen hastalar ortalama 13 (6-15) ay takip edilmiştir. Konservatif olarak tedavi edilen anstabil kırıklı 26 hastanın 20’sinde (%76.9) iyi, 6’sında (%23.1) kötü; cerrahi olarak tedavi edilen 3 (%16.6) olguda kötü, 15 (%83.4) olguda iyi sonuç alınmıştır. 4 (%22.2) olguda rotasyon kusuru, 1 (%5.5) olguda redüksiyon kaybı oluşmuştur. Anstabil kırıklarda cerrahi tedaviyle iyi sonuç alma oranı konservatif tedaviye göre daha fazladır. 11-16 yaş grubundaki anstabil kırıklarda rotasyon kusuru oluşma oranı, stabil kırıklara göre çok daha fazladır. Cerrahi tedavide redüksiyon kaybı oluşma oranı konservatif tedaviye göre çok daha azdır. Kapalı intramedüller tedavi hem komplikasyon riskinin yüksek olmaması, hem de iyi sonuç alınması açısından en uygun tedavi yöntemi olarak görülmektedir.
